ASOC Senate Funding Request Form- Spring 2009

Fill out the following information completely. Any incomplete information could delay your request and potentially decrease the percentage awarded. Please attach additional information as appropriate.

This form is due by every Thursday at 5:00 PM. The Funding Request Meeting is every Friday at 12:30 PM in the ASOC Office in the Johnson Student Center. Attendance is required. All funding requests must be submitted and approved by Senate prior to the event's date. There will be no retroactive funding.

All events sponsored by ASOC must have "ASOC" on their promotional materials.

**PLEASE NOTE: All receipts for funded projects must be received by the ASOC Finance Office by May 1.**
